Home Assessments – Multidisciplinary Analysis Report
Our home assessments are designed to provide a comprehensive and multidisciplinary analysis report of the entire environment in which the injured worker resides.
The nurse case manager reviews multidisciplinary medical reports, interviews the injured worker, and assesses the following:
- Level of assistance with Activities of Daily Living (ADL)
- Home modification needs
- Nutrition needs
- Mobility issues
- Hygiene conditions
- Social and economic issues
