Independent Nurse Case Management Services
Established in 2001 under the name of Care Connection, Inc., In January 2011, the organization changed its officers and name to IW Care Connection, Inc. (IWCC). In May 2013, IWCC was certified as a Women’s Business Enterprise (WBE) and a Small Business Enterprise (SBE) by the Los Angeles County Metropolitan Transportation Authority (MTA).
IWCC’s registered nurses and certified case managers are knowledgeable in all aspects of workers’ compensation and the majority of them are bilingual in Spanish. The IWCC team has extensive experience working with insurance companies, self insured employers, third party administrators, applicant attorneys, and defense attorneys. The Nurse Case Manager handles all necessary tasks needing to be accomplished and reports back to all parties.
CASE MANAGEMENT FUNCTIONS
IW Care Connection Provides Include:
• Comprehensive Home Assessments to determine In-home medical needs
• Facilitates and coordinates medical care on accepted diagnoses
• Monitors the quality of care as well as outcomes
• Attend physician appointments as deemed necessary
• Telephonic Case Management if preferred by the parties
• Catastrophic Care Management
• Confers with physicians for clarification of medical issues and reports
• Discharge planning after hospital or skilled nursing facility discharge
• Arranges for authorized treatment, transportation, and durable medical equipment (DME)
• Locate facilities and oversee home health care agencies for authorized care
• Coordinate all case issues with the adjuster and attorneys
• Communication and coordination with all ancillary providers
• The case manager is the core that sees that the care of the injured worker
is monitored so that all the case needs are met until the case is settled
