The Nurse Case Manager coordinates and manages medical and non-medical needs for the injured worker and reports back to all parties. Our Registered Nurses, facilitate and coordinate medical care for accepted diagnoses, attend physician appointments as deemed necessary, confer with physicians regarding clarification of medical issues, coordinate the discharge planning after hospital or skilled nursing facility, locate facilities and oversee home health care agencies, arrange for authorized treatment, transportation, and DME.
Comprehensive Home Assessment
Home Assessments are designed to provide a comprehensive report of the entire environment in which the injured worker resides. Functions the Nurse Case Manager performs during the home assessment include:
- Evaluation of medical status for functional needs/limitations
- Evaluation of social economic conditions at the home
- Evaluation of activities of daily living (ADLs) performance
- Evaluation of household management conditions
- Evaluation for home health and/or caregiver needs
- Evaluation of home modification needs and assistive equipment
Catastrophic Nurse Case Management
Catastrophic injuries present immediate and long term cost risks for the entire industry in which the injured worker was involved. Without proper management, recovery can be prolonged and more costly than necessary. IW Care Connection, Inc., utilizes expert CAT Nurse Case Managers countrywide with relationships and access to Regional Trauma Centers and Centers for Excellence in various types of health and wellness treatments. Our CAT (computerized axial tomography) Nurse Case Manager ensures appropriate and timely care for the injured person while conveying care and concern to the patient and family. Catastrophic injuries requiring emergent care and potential for sub-acute care are:
- Spinal cord injuries and diseases including quadriplegia, paraplegia and paresis
- Burns (1st, 2nd, 3rd degree; full thickness)
- Traumatic brain injuries including close head injuries with loss of consciousness
- Bilateral vision loss
- Multi-trauma (two or more substantial injuries)
- Chemical respiratory
Ergonomic Evaluation
Ergonomic Evaluation is a work station or task evaluation designed to measure a job’s potential for injury or fatigue. The evaluation is based on the applied science of ergonomics, which maximizes productivity by designing equipment and workflow to reduce operator fatigue and discomfort. Standardized risk assessment tools are used to quantify risk. The evaluation takes place in both industrial and office settings.
Effective injury prevention programs are based on employer/employee education and a sound ergonomic evaluation program. Successful programs work within production goals of the facility while guarding the safety and well-being of the worker.
